America´s 1st Choice is a health Plan with a Medicare contract. This contract is renewed annually, and the availability of coverage beyond the end of the current contract year is not guaranteed. Medicare approved Medicare Advantage Private Fee for Service plans are available to anyone entitled to Part A and enrolled in Part B of Medicare through age or disability, not in ESRD, and in an approved service area. Members may enroll in the plan only during specific times of the year. Contact the plan for more information. You must continue to pay your Medicare applicable premiums if not otherwise paid for under Medicaid or by another third-party. Plans may be renewed annually. All plan types may not be available in all areas. Benefits vary by plan and counties. Benefits, Formulary, Pharmacy network, Premium and or co-payments / coinsurance may change on Jan: 1 2013. We are required to inform you by law, that we can choose not to renew our contract with CMS, reduce our service area or no longer offer services in the area where you reside. CMS may also refuse to renew our contract, thus resulting in a termination or non-renewal. Should this occur, this may result in termination of your enrollment in the plan.
